Think about it this way for a second

Stale is like fish that's gone off — it might have been fine yesterday, but it's bad today. The syllabus got revised, the rules changed, you fixed your own mistake — but if the old version is still sitting on the table, the model will read the old one. That's why every file needs a date and a heading. If two files disagree, go with the newer one. If you're not sure, just ask.

If you have both phones-2025.md and phones-2026.md, don't include the 2025 one. Don't treat a wrong answer from earlier in the chat history as evidence. Try this — feed in one old-dated item and one new-dated item, and see which one it latches onto. If it latches onto the old one, add "only use the latest date" to your card.
